Appointments
Our patients are seen by appointment only. Office hours are from 8:00 AM to 5:00 PM, Monday through Friday.
Despite our best efforts to see patients on schedule, unforeseen emergencies may occur, causing delays. We will try to keep you informed about any changes in our appointment schedule.You can help us maintain our schedule by being on time for your appointment. Should you have to wait, please know that we regret any inconvenience and ask for your understanding.
Preparing for your visit
We ask that you arrive 15 minutes prior to your first office appointment. During this time we will ask you to complete a patient information form which requires:
- General/Personal Information
- Brief medical history, with medications and allergies
- Previous lab work, X-rays, surgical reports, or EMGs
- Insurance information

If you are being seen for a consultation, second opinion, or an independent medical examination, be sure to bring any X-rays and/or records available from any prior treatment. This will help the doctor to thoroughly complete your evaluation.
If your writing hand is temporarily disabled, you may consider asking a family member or friend to accompany you to your first appointment. In the event that this is not possible, please do not hesitate to request assistance from our office staff.
