Our patients are seen 8:00 a.m. to 5:00 p.m., Monday through Friday, by appointment only. Despite our best efforts to see patients during their scheduled time, unforeseen emergencies can occur causing delays. We will do our best to keep you informed of any changes to your scheduled appointment. We appreciate your help in maintaining our schedule by being on time. If you have to wait, please know that we regret any inconvenience and ask for your understanding.

New Patient Forms
New patients should arrive 15 minutes prior to your first office appointment. During this time you will need to complete the following patient information forms:
- General/Personal Information
- Brief medical history, with medications and allergies
- Previous lab work, X-rays, surgical reports, or EMGs
- Insurance information

If you are being seen for a consultation, second opinion, or an independent medical examination, be sure to bring any X-rays and/or records available from any prior treatment. This will help the doctor to thoroughly complete your evaluation. If your writing hand is temporarily disabled, you may consider asking a family member or friend to assist you in completing the forms prior to your visit or have them accompany you to your first appointment.
