Please verify we are an in-network provider with your insurance prior to your appointment. You must bring your current insurance card and be prepared to make your co-payment at the time of your visit. If your insurance requires you to have a referral, call your Primary Care Physician as soon as you schedule your appointment. If you are unsure whether your insurance requires a referral, please call the number on the back of your insurance card.
If you’re being seen for a Worker’s Compensation injury, you’ll need the name and address of your employers Worker’s Compensation Carrier, along with an assigned claim number. If you do not have this information at the time of your appointment, we will reschedule you or bill you as self-pay. If you have no health insurance, please be prepared to make the required payment toward your initial visit. Payment arrangements for subsequent charges can be made with the billing department.
If you are being seen for a second opinion, please bring all x-rays, MRI’s and other tests that have been done previously. In addition, we will need all medical records from the original treating physician relating to the problem for which you are seeking a second opinion. Bring x-rays, bone scans, MRI’s or EMG’s that have previously been done regarding the problem that you will be treating for. This means written reports as well as actual films. Please call the facility where these tests were performed at least 24 hours in advance in order to obtain this information.
